Job Title: Stakeholder and Relationship Manager - IAR
Job Type: Contract 12 months + possible extension(s)
Location: Downtown Toronto, ON
Rate: Flexible based on experience



This government client is seeking an experienced Stakeholder and Relationship Manager to plan and manage project teams in the development of a strategic implementation plan to support the delivery of concurrent Health Service Providers (HSP) implementations.


Responsibilities and Deliverables:

The Stakeholder and Relationship Manager - IAR will be required to:


Manage the development and execution of a strategic implementation plan which leverages the provincial asset, Integrated Assessment Record (IAR), for provincial care coordination across health care sector HSPs. The implementation will align with the government's objectives for evidence-based patient care planning and management;
Work with established Working Groups to develop a configurable solution that will meet their business needs, and manage the adoption of HSPs and implement IAR for care coordination;
Provide senior level expertise and advice regarding project, business and resource management impacting the care coordination implementation plan, day to day operations and other strategic initiatives. Develop and implement recommendations to resolve issues in support of project delivery, timelines, scope and budget;
Work with other project managers to support the use of industry best practices with regards to release management, change management, incident management, problem management, privacy and security breach management for the IAR operational environment;
Provide project management expertise, strategic advice and support to the Special Projects Branch to ensure effective governance, risk management and mitigation strategies and recommended approaches and plans are in place to support project success;
Lead the development and execution of strategic communications for the project; and foster collaborative relationships with internal and external stakeholders, HSPs and third party vendors to ensure coordinated and effective communications that are aligned with ministry and government priorities;
Liaise with the Ministry, Health Service Providers, Local Health Integration Networks (LHINs), and other supporting governance structures to review and discuss issues, seek approval on new requests and to provide reports related to the implementation status, operational activities, and other key deliverables;
Provide strategic input and recommendations to current and future initiatives and manage other operational activities and project duties as required;
Development of integrated project plans that ensure accurate and appropriate inclusion of deliverables, milestones, resources, inter-project dependencies and progress reporting;
Lead the development of feasibility studies by ensuring the quality of business cases are developed following the Ministry's framework;
Lead the development and management of governance structures and working groups;
Provide demonstrations and presentations of care coordination progress and solutions to a variety of audiences including Ministry, LHINs and other stakeholders;
Delivery of business applications to support the care coordination projects and enable care coordination processes;
Evaluations of care coordination outcomes and recommendation of Provincial strategies based on implementation results;
Financial and resource management plans; and
Communication documents, presentations, reports and briefing materials.


Skills and Experience:


Project Management:

Demonstrated experience managing large and complex projects with a demonstrated track record of delivering successful business outcomes;
Proven senior leadership and resource management skills including the ability to build high performing teams and manage a variety of professionals;
Strong working knowledge of project methodologies and standards (e.g. PMBOK) and tools,(e.g. MS Project);
Track record of successful project delivery within approved plan, scope and budget;
Sound knowledge of finance and project cost-accounting principles and practices with demonstrated experience with managing project budgets.
Business Knowledge:

Demonstrated experience managing business projects and achieving successful results on time and on budget with high customer satisfaction;
Understanding of and/or experience working with care coordination, common assessment tools and their applications in the Community Care sectors and the broader health system;
Demonstrated experience in applying technical solutions to address business objectives in the integration and management of data;
Understanding of and or experience with the maintenance of information standards involving multiple stakeholders.
Stakeholder Relationship Management :

Superior consultation, negotiation, and consensus building skills to work collaboratively and build consensus with internal and external stakeholders and business partners;
Excellent communication and problem-solving skills, exceptional judgement and political acu
