Ontario is improving co-ordination of care for high-needs patients such as seniors and people with complex conditions through the creation of up to 98 Health Links across the province. Health Links will encourage greater collaboration and coordination between different HSPs, as well as the development of personalized care plans.
Supporting the Transformation Secretariat initiative for Health Links, our client initiative is positioned to support the delivery of business solutions across multiple health sectors.
Leveraging the Integrated Assessment Record (IAR) component for provincial care coordination will enable HSPs in a given geographic area (family doctor, hospital, home and community care providers) to work together to provide coordinated health care plans for individual patients. This will help improve patient transitions within the system and help ensure patients receive more responsive care that addresses their specific needs with the support of a tightly knit team of providers.
The Business Transformation Analyst - CCT Implementation will be required to:
•Work with subject matter experts and end users of the HSPs on validation of business requirements and related data definitions or specifications; Identify and translate high level, conceptual user-requirements into business requirements and identify procedures, decision flows and develop associated data dictionaries for the implementation of assessment sharing technology;
•Create and manage documentation of business requirements, or other required documents to support the development of processes related to the sharing and viewing of information of a patient-centered care plan;
•Support the vendor throughout the implementation process, including executing process modeling activities, reviewing and providing input to the impact of any process changes, developing business use cases to support information sharing processes and integration of a patient-centered care plan into standard business practices across a variety of sectors;
•Advise and lead the development of acceptance scenarios for the business application and ensure the completion of the acceptance cycle with business users;
•Analyze processes and business practices related to the IAR suite of products and identify any business requirements or process improvements and their related impacts to end users;
•Work directly with the vendor and other stakeholders to analyze business processes and translate functional requirements into business language, and develop plain language documentation that is understood by internal and external stakeholders (including technology teams);
•Foster collaborative relationships with internal and external stakeholders, including HSPs and third party vendors to ensure coordinated and effective communications that are aligned with Ministry and government priorities;
•Support the Ministry's and LHINs' governance structure to review and discuss issues regarding data standards and data integration, approval on new requests and provide reports related to the implementation status, operational activities, and other key deliverables;
•Provide strategic input and recommendations to current and future initiatives and manage other operational activities and project duties as required; and
•Work with other Business Transformation Analysts to support the use of industry best practices with regard to release management, change management, incident management, problem management, privacy and security breach management for the CCT environment.
Examples of deliverables include:
•Development of user and business requirement documentations and associated data dictionaries;
•Development of process models and use cases; and
•Communication documents, presentations, reports and briefing materials.
•Demonstrated experience with implementation methodologies and processes related to health care sector change projects;
•Experience in performing business analyses of functional requirements to identify information, procedures and decision flows and develop associated data dictionaries;
•Ability to independently determine and manage tasks and processes for the purposes of meeting high level deliverables ;
•Demonstrated experience in applying technical solutions to address business objectives in the integration and management of data;
•Understanding of and or experience with the maintenance of information standards involving multiple stakeholders;
•Demonstrated experience working collaboratively with internal and external stakeholders and business partners ;
•Demonstrated experience resolving conflicts and major obstacles;
•Demonstrated ability managing cross-functional project teams and effectively working in a matrix environment.
